Reserpine is an alkaloid isolated from the root of the snakeroot plant (Rauwolfia serpentina), a small evergreen climbing shrub of the dogbane family native to the Indian subcontinent. Known in India as Sarpaganda, it has been used for centuries to treat hypertension, insanity, snakebite, and cholera due to its antipsychotic and antihypertensive properties.

Reserpine irreversibly binds to the storage vesicles of neurotransmitters, particularly norepinephrine, serotonin and dopamine causing catecholamines to leak into the synapse so that they are not available for release when the pre-synaptic neuron is stimulated. Eventually, catecholamine depletion occurs because of the body's inability to store these neurotransmitters. The antihypertensive actions of reserpine are a result of its ability to deplete catecholamines from peripheral sympathetic nerve endings. These substances are normally involved in controlling heart rate, force of cardiac contraction and peripheral vascular resistance and thus, result in a reduction in both cardiac output and peripheral vascular resistance with long term therapy, which takes approximately 3 weeks to develop after the initiation of therapy. 

The chief use of reserpine is for treating mild essential hypertension. It is also useful as adjunctive therapy with other antihypertensive agents in the more severe forms of hypertension. It is used in cases of mild anxiety and to treat severe agitation in patients with mental disorders (e.g., schizophrenia), primarily in those individuals unable to tolerate phenothiazine derivatives or in those who also require antihypertensive medication.
